Current therapies for breast cancer include treatments that are toxic and often result in drug resistance. Telomerase is thus an attractive target for both diagnosis and therapy because of its distinct pattern of expression. We address the use of telomerase in the diagnostics of breast pathology, as well as the use of telomerase inhibitors in the treatment and prevention of breast cancer. Human chromosomes contain repeated TTAGGG DNA sequences at their ends telomeres that provide genomic stability protect the ends from being recognized as DNA breaks needing repair and a source of expendable DNA a solution to the inability of the replication machinery to copy the extreme ends of chromosomes: the end replication problem see [ 1 ] for a review. A significant amount of truncation can occur during DNA replication without causing deleterious effects. Human telomeres progressively shorten with ongoing cell division until they reach a critical length that induces replicative senescence [ 2 ].

Telomeres and telomerase are the pivotal players that destine cellular senescence and immortalization. Telomere shortening due to "End-replication problem" in each cell cycle is a universal phenomenon for all eukaryotic cells with linear chromosomes. Its compensation by telomerase is also common from yeast to human. However, the length of telomeres, distribution of telomerase positive cells, and consequence of telomerase inhibition are different among species. Cutting-edge experimental protocols for their evaluation are also included. This coverage will enable medical oncologists and students as well as basic researchers to capture the up-to-date entire picture of human telomeres and telomerase in cancer.

Despite a strong correlation between telomerase activity and malignancy, the outcome of telomerase inhibition in human tumor cells has not been examined. Here, we have addressed the role of telomerase activity in the proliferation of human tumor and immortal cells by inhibiting TERT function. Inducible dominant-negative mutants of hTERT dramatically reduced the level of endogenous telomerase activity in tumor cell lines. Clones with short telomeres continued to divide, then exhibited an increase in abnormal mitoses followed by massive apoptosis leading to the loss of the entire population. This cell death was telomere-length dependent, as cells with long telomeres were viable but exhibited telomere shortening at a rate similar to that of mortal cells. It appears that telomerase inhibition in cells with short telomeres lead to chromosomal damage, which in turn trigger apoptotic cell death.
